Blood-streaked sputum: what to know

A streak can follow a forceful cough, a respiratory infection, or irritation from smoke and dry air. Acute bronchitis and pneumonia are possible causes. Bronchiectasis and COPD may cause recurring blood-streaked sputum, particularly when there is a change in the usual cough or mucus. Bleeding from the nose, gums, or throat can also be mistaken for sputum from the lungs.

Most people with these conditions have a broader pattern of symptoms, so a streak alone cannot tell you which condition is present. Describe fever, breathlessness, chest discomfort, weight change, smoking history, and medicines that affect clotting.

Take a clear note of the colour, amount, and frequency, and whether the blood is mixed through mucus or sits on its surface. Seek medical advice if it recurs, has no obvious upper-airway source, or occurs with an ongoing cough. Avoid smoking and seek advice before using any new medicine; antibiotics are not needed for every cause.

Use emergency services for increasing bleeding, persistent bleeding, more than a trace, shortness of breath, chest pain, faintness, confusion, or blue-grey lips. Arrange urgent assessment for blood with high fever, severe weakness, or a sudden deterioration. Report medicines such as anticoagulants or antiplatelets without stopping them on your own.

Ask whether the pattern suggests an upper-airway or lower-airway source, what examination or tests may help, and how long to watch for recurrence. Ask for a clear action plan if the amount increases or breathing changes.
