What to know first

Check that the blood is vaginal rather than coming from urine, the rectum or a skin injury. Pink, red or brown staining can all be vaginal blood; colour alone does not reveal why it happened.

Causes clinicians consider

Hormonal contraception, irregular ovulation, early pregnancy and small injuries after sex can alter bleeding patterns. Chlamydia or another infection may inflame the cervix. Endometrial polyps and uterine fibroids can also cause bleeding between expected periods.

Changes in cervical cells, cervical cancer and womb cancer can present with unexpected vaginal bleeding. Most importantly, spotting cannot show which explanation applies. Your age, pregnancy possibility, menstrual pattern, medicines, examination findings and tests guide the diagnosis.

Write down when the spotting started, how much there was and whether it followed sex. Add any missed period, pelvic pain, unusual discharge, urinary or bowel symptoms, contraception changes and medicines that affect bleeding. If pregnancy is possible, use a home pregnancy test as directed.

Arrange an appointment if the episode has no clear minor trigger, repeats, continues, appears after sex or occurs after menopause. The visit may include a pregnancy test, infection testing, examination of the vagina and cervix, cervical screening when appropriate, blood tests or ultrasound.

When not to wait

Use emergency services for very heavy bleeding, fainting, severe weakness, breathing difficulty or intense abdominal or pelvic pain. When pregnancy could be possible, spotting with one-sided pain, shoulder-tip discomfort, collapse or significant dizziness requires immediate evaluation.

Seek prompt advice for fever, foul-smelling discharge or escalating pelvic pain, which may accompany an infection. Bleeding after menopause should also be checked promptly, even when it is only a stain and has already stopped.
