Blood in phlegm without coughing

Post-nasal drainage can carry blood from dry or irritated nasal tissue into mucus. Bleeding gums, mouth ulcers or a small throat injury can look similar after spitting. Acid reflux may inflame the throat, and a cold or allergic rhinitis can make secretions thicker and easier to notice. Airway conditions such as bronchiectasis can also present with blood-streaked sputum; the absence of coughing does not establish the source or rule out a chest problem.

Notice whether the material is clear mucus, thick sputum or saliva, and whether it follows brushing, nose blowing or waking up. Keep a simple record and photograph the tissue only if useful for showing a clinician. Avoid picking the nose, smoking and repeated hard clearing of the throat. Book an appointment when the symptom recurs, persists, or occurs while using medicine that affects clotting.

Use emergency services for severe breathing difficulty, chest pain, collapse, confusion or bleeding that will not stop. Seek prompt assessment for repeated episodes, increasing volume, fever, unexplained weight loss, night sweats or new breathlessness. If you feel weak or light-headed, sit down and have someone stay with you while arranging help.
