Why a facial lump may contain blood

An epidermoid cyst is a sac under the skin that usually holds keratin. Blood may appear after squeezing, shaving, friction, inflammation, or rupture. Acne nodules, boils, blood blisters, and vascular growths can also be mistaken for a blood-filled cyst, particularly when swelling hides the original surface features.

Location matters during assessment because a lump near the eye, nose, lip, or facial nerve requires careful planning. The clinician may check for a central opening, mobility beneath the skin, warmth, fluctuance, discharge, and nearby swelling. These findings help distinguish a cyst from infection or another skin lesion.

Care without worsening the area

Stop squeezing or needling the lump. Cleanse with a mild product, avoid scrubs and strong acne treatments on broken skin, and keep razors and makeup away from an open surface. A warm, clean compress may feel soothing, but forceful pressure can drive inflammation deeper and increase scarring.

For active bleeding, hold clean gauze firmly against the site rather than repeatedly lifting it to look. Once bleeding has settled, protect the spot with a small non-stick dressing. Take a photograph before covering it if doing so does not delay care; this can show the clinician how it looked before drainage or crusting.

When an examination is needed

Arrange prompt care if redness or swelling spreads, pain intensifies, pus develops, fever occurs, or swelling approaches the eye. A facial lump should also be checked if it grows, repeatedly bleeds, remains open, changes shape or colour, or returns after being emptied.

Urgent help is appropriate for difficulty seeing, marked swelling around the eye, trouble breathing or swallowing, severe systemic illness, or bleeding that continues under steady pressure. Do not delay assessment because the lump was previously labelled a cyst if its behaviour has changed.

Ask whether inflammation should settle before a procedure, whether antibiotics are appropriate only if infection is present, and how the location affects scar placement. If removal is proposed, clarify whether the entire cyst wall will be taken out and whether the tissue will be examined.
