The blob sign and ectopic pregnancy

It describes a mass seen next to an ovary that appears separate from the ovary. The finding must be interpreted with the entire scan, pregnancy hormone results, pregnancy dates, symptoms and repeat assessment when needed.

An ectopic pregnancy develops outside the main cavity of the uterus, most often in a fallopian tube. On an early scan, the pregnancy may be too small to show definitive structures, so clinicians consider the location of any mass and whether a pregnancy is visible inside the uterus.

A blob sign is concerning but is not interpreted in isolation. Ovarian structures and other pelvic findings can resemble an adnexal mass, and the sonographer or clinician correlates appearances with blood tests and changes over time.

Follow the early-pregnancy team's instructions and make sure they know about any pain, bleeding, dizziness or shoulder-tip pain. Depending on the complete assessment, management may involve close monitoring, medicine or surgery; the appropriate route depends on stability, scan findings, hormone patterns and individual circumstances.

Keep the agreed blood-test and scan appointments even if symptoms settle. Avoid travelling far from emergency care until the clinical team has clarified the pregnancy location and explained what changes require immediate help.

Call emergency services or go to an emergency department now for severe or increasing abdominal or pelvic pain, collapse, fainting, marked dizziness, shoulder-tip pain, difficulty breathing, confusion, pallor or heavy bleeding. These can accompany internal bleeding from a ruptured ectopic pregnancy.

Do not wait for a scheduled scan or blood test when these symptoms develop. If pregnancy is possible but has not been confirmed, tell the emergency team about the possibility.

Ask what the scan showed inside the uterus and beside each ovary, whether the pregnancy location is confirmed, what follow-up is planned, which treatment options apply, and exactly where to go if pain or bleeding changes.
