Blisters and psoriasis

A fluid-filled area in someone with psoriasis may instead reflect rubbing, a burn, contact irritation, infection, or another skin problem. Do not assume every new blister is part of an existing diagnosis, particularly when it looks or feels different from the usual patches.

Plaque psoriasis commonly presents with clearly bordered, raised areas and scale. Dryness and cracking can sometimes release clear fluid or blood, which may be mistaken for a blister. Pustular psoriasis presents with small pus-filled bumps on inflamed skin; it may stay on the palms and soles or affect a wider area.

The fluid in pustular psoriasis is not by itself proof of a bacterial infection. Even so, infection can also cause pus, warmth, tenderness, crusting, or spreading redness, so a clinician may need to examine the skin. Itch, peeling, redness, and scaling describe the surface but do not establish the cause.

Protect a clear blister with a clean non-stick dressing and reduce rubbing. Do not squeeze pus-filled bumps or peel loose scale. Continue prescribed psoriasis treatment only as directed; avoid applying a strong medicated cream to an unfamiliar blister unless the prescriber has said it is suitable for that location.

Take photographs as the eruption changes and record recent illness, missed or changed medicines, skin injury, and any new topical product. Contact the clinician managing your psoriasis because treatment depends on whether this is pustular disease, irritation, infection, or an unrelated blister.

Seek urgent care when pus-filled bumps spread across large areas, especially with fever, chills, marked weakness, rapid heartbeat, or feeling severely unwell. Widespread redness, tenderness, and peeling also warrant urgent assessment because extensive pustular psoriasis can affect more than the skin.

Obtain prompt advice for increasing warmth, swelling, pain, drainage, red streaks, or fever, which can accompany infection. Blisters involving the eyes or mouth, breathing difficulty, facial swelling, or a painful peeling rash after a new medicine require emergency evaluation.

Describe whether the bumps contain clear fluid or pus, where they began, how quickly they spread, and whether your established psoriasis has changed. Ask if the eruption fits pustular psoriasis, whether infection must be excluded, how current medicines should be handled, and which skin-care products are appropriate while it heals.
