Bleeding With Intercourse After Hysterectomy

Stop intercourse when bleeding occurs. If surgery was recent, contact the surgical team promptly; if recovery was completed long ago, arrange a gynaecology assessment rather than assuming the bleeding is expected.

The source depends partly on which structures were removed and how long ago surgery occurred. During recovery, the tissue closure at the top of the vagina can be delicate. Later bleeding can arise from dryness, friction, inflammation, a small area of healing tissue, infection, or a lesion of the vulva or vagina.

If the cervix remains after a subtotal hysterectomy, cervical tissue can also bleed and still requires appropriate screening. Pelvic Pain with sex or unusual discharge provides useful diagnostic information but does not determine the cause by itself.

Pause penetrative sex until you have advice, especially while recovering from surgery. Use a pad to observe the amount and colour; avoid tampons or inserting products into the vagina if healing is incomplete. Record whether bleeding happens every time, whether penetration is painful, and whether there is dryness, odour, discharge, urinary discomfort, or pelvic pressure.

Tell the clinician the type and date of hysterectomy, why it was performed, whether the cervix was retained, and any treatment that followed. An examination can check the vaginal walls and surgical site and guide whether swabs, imaging, or other tests are needed.

Seek urgent care for heavy or increasing bleeding, severe abdominal or pelvic pain, fainting, marked weakness, fever, vomiting, foul-smelling discharge, or a feeling that tissue is bulging from the vagina. After recent surgery, sudden pain or a watery discharge also warrants immediate contact with the surgical team.

For light bleeding that stops and no warning symptoms, avoid further penetration and arrange an appointment. Recurrent spotting should not be managed only with lubricant without first confirming its source.

Ask whether the vaginal surgical site has healed, whether the cervix remains, and whether dryness, inflammation, infection, or healing tissue is visible. Discuss when intercourse can safely resume, which lubricant or vaginal moisturiser is suitable, and whether any screening follow-up remains necessary.
