Bleeding during intercourse after hysterectomy

Early in recovery, delicate healing tissue may bleed if penetrative sex resumes before the surgical team has cleared it. Later bleeding can arise from vaginal dryness, friction, infection, granulation tissue at the vaginal cuff, a small tear, or another vaginal or cervical condition if the cervix remains.

The relevant anatomy depends on whether the uterus alone was removed or the cervix was removed as well. When the cervix is removed, the top of the vagina is closed to form a vaginal cuff. The ovaries may remain and continue producing hormones, while removal of both ovaries can contribute to vaginal dryness.

A clinician may inspect the vulva, vagina, cuff, and any remaining cervix. They will consider the timing of surgery, pain, discharge, odour, dryness, medicines, and the reason for hysterectomy before deciding whether swabs, imaging, or tissue sampling is needed.

Stop penetrative sex if it triggers pain or bleeding. During postoperative recovery, follow the surgeon's restrictions and contact the surgical team rather than testing whether the area has healed. If recovery is complete, book a gynaecology review for bleeding that recurs or is more than a faint isolated spot.

Record the amount, colour, pain, discharge, and whether bleeding happens every time. Once serious causes and incomplete healing have been excluded, a clinician may suggest lubricant, a vaginal moisturiser, or treatment tailored to infection, dryness, or granulation tissue.

Seek urgent care for heavy or increasing bleeding, severe pelvic or abdominal pain, fever, foul-smelling discharge, dizziness, fainting, or tissue protruding from the vagina. Sudden pain or bleeding during early recovery may indicate disruption of the vaginal cuff and needs prompt examination.

Call emergency services if bleeding is rapid, you collapse, or you feel too weak or breathless to travel safely. Do not insert anything into the vagina while awaiting assessment.

Ask whether your cervix and ovaries were retained, whether the vaginal cuff is fully healed, whether screening remains relevant for you, what is causing the bleeding, and which sexual activities can be resumed safely.
