The short answer
Light spotting may occur around ovulation or after a change in hormonal contraception. Infection, pregnancy-related problems, fibroids, endometriosis and adenomyosis can also present with bleeding and pelvic pain, so the timing and associated symptoms matter.
What the combination may mean
A brief episode near the middle of a cycle can accompany ovulation. Bleeding after sex may come from irritation of the vagina or cervix, while unusual discharge, fever or pain during sex can point toward a pelvic infection. Fibroids may present with pressure and abnormal bleeding; endometriosis or adenomyosis may present with recurring pelvic pain and difficult periods.
Bleeding in early pregnancy can occur for different reasons, but bleeding together with one-sided pain, shoulder-tip pain, dizziness or faintness needs urgent assessment because a pregnancy outside the uterus must be excluded.
What to do now
Record when the bleeding started, its colour and flow, where the cramp is felt, and whether it follows sex or occurs at a similar point in each cycle. Note contraception, missed tablets, medicines, discharge, urinary symptoms and any chance of pregnancy. Use a home pregnancy test if pregnancy is possible, following its instructions.
Arrange a clinical appointment if the episode returns, continues, or differs from your usual pattern. An assessment may include a pregnancy test, examination, infection testing, blood tests or pelvic imaging, chosen according to your history rather than performed automatically.
When to seek urgent care
Seek urgent medical help for heavy bleeding that rapidly soaks protection, severe or worsening pelvic pain, fainting, marked dizziness, weakness, fever, vomiting, shoulder-tip pain, or bleeding with a positive pregnancy test. Bleeding after menopause also warrants prompt medical assessment, even when it is light.
If symptoms are mild but keep returning, book a routine appointment rather than waiting for the next episode to become more intense.
