Bleeding after sex postpartum following a caesarean birth

A caesarean incision is on the abdomen and uterus, yet postpartum blood and tissue still leave through the vagina while the uterus heals. Sex can make existing flow more noticeable without proving that penetration caused it. Labour before the operation, vaginal examinations or an assisted delivery attempt may also leave tender vaginal or cervical tissue.

Even without vaginal trauma, postpartum hormonal changes can cause dryness. Friction against dry tissue may create a small surface graze. Less commonly, bleeding with pelvic pain, fever or offensive-smelling discharge can accompany an infection, while bleeding that repeatedly follows sex may arise from the vagina or cervix and merits examination.

Stop penetrative sex and monitor the bleeding with a maternity pad. Check whether the abdominal wound is becoming more painful, red, swollen or leaky, although vaginal bleeding does not usually come through that incision. Follow your maternity team's advice about resuming sex and allow more recovery time if bleeding, discharge, wound discomfort or fatigue continues.

When you have been cleared and feel ready, begin gently, choose a position that avoids pressure on the abdomen and use water-based lubricant if needed. Seek a postpartum review if bleeding recurs after intercourse, pain persists, or you are unsure whether normal postpartum flow had finished. A clinician can inspect vaginal and cervical tissue and assess the uterus and caesarean wound.

Urgent assessment is needed for sudden heavy or gushing bleeding, large clots, severe or worsening abdominal pain, fainting, marked dizziness, a racing heartbeat, breathlessness or feeling acutely unwell. Contact maternity care promptly for fever, chills, foul-smelling vaginal discharge or worsening tenderness around the uterus or incision. Call emergency services for collapse, chest pain or severe trouble breathing.

Tell the clinician when the caesarean occurred, whether labour began first, how postpartum flow changed, and exactly when sex was resumed. Ask whether your uterus and incision are healing as expected, whether vaginal dryness needs treatment, whether examination or infection testing is appropriate, and which activities to pause.
