Bleeding after first sex: what to know

Some people bleed and others do not; either experience can occur. Bleeding is not proof of virginity, and the appearance of the hymen cannot establish sexual history.

Penetration can stretch delicate tissue at the vaginal opening or create a small surface graze, especially when lubrication is limited, arousal is brief, or movement is forceful. Blood may also come from a period that is beginning or ending.

Bleeding that returns after sex can have other explanations, including vaginal dryness, cervical irritation, a polyp, or an infection such as chlamydia. Postcoital bleeding can be a presentation of cervical or vaginal disease, so repeated episodes warrant an examination rather than an assumption about the cause.

Pause penetration until bleeding and tenderness have settled. Use a pad or liner to see how much blood there is; avoid inserting a tampon into sore tissue. Gently wash the outside with water and avoid douching or scented products.

For future sex, allow more time for arousal, use a suitable lubricant, begin slowly, and stop if there is sharp pain. Condoms help reduce sexually transmitted infection risk, while contraception is needed if pregnancy is not wanted.

Get urgent medical help if bleeding is heavy, you feel faint or weak, pain is severe, or you may be pregnant and have pelvic pain. Seek prompt care for fever, foul-smelling discharge, a deep tear, or bleeding that does not ease.

Arrange a routine appointment if bleeding happens again after sex, occurs between periods, or comes with unusual discharge, burning, or pelvic discomfort. A clinician may examine the vagina and cervix and offer pregnancy, infection, or cervical screening tests according to your circumstances.

You might ask where the bleeding appears to come from, whether testing for pregnancy or sexually transmitted infections is appropriate, and whether cervical screening is due. Mention the amount and colour of blood, pain, discharge, period timing, contraception, medicines, and whether sex was consensual.
