Bleeding After 2 Years Without a Period

Do not wait for another episode before arranging care. Record when the bleeding began, how much there was, and whether it followed sex or came with pain.

If those two years followed your final natural period, this is postmenopausal bleeding. Possible explanations include fragile vaginal or womb-lining tissue after lower oestrogen levels, a cervical or uterine polyp, changes in the womb lining, infection, or an effect of hormone therapy or another medicine. Cancer of the womb or cervix is a less common but serious possibility that must be excluded.

A long gap without periods does not always confirm menopause. Age, pregnancy possibility, contraception, hormone treatment, previous surgery, and conditions that affect ovulation all change how a clinician interprets the bleeding.

Arrange a prompt appointment with a gynaecologist or primary-care clinician. Mention every episode, including pink or brown discharge, and bring a list of medicines, supplements, hormone therapy, and contraception. If pregnancy is possible, say so because it changes the tests and immediate care.

Assessment may involve an examination of the vulva, vagina, and cervix, followed by a transvaginal ultrasound and sampling of the womb lining when appropriate. The tests are chosen according to your history and findings; treatment targets the identified cause.

Seek emergency care if bleeding is heavy or rapidly soaking pads, or if it occurs with fainting, marked dizziness, breathlessness, chest pain, severe pelvic or abdominal pain, confusion, or clammy skin. These signs can indicate significant blood loss or another urgent problem.

Without those emergency signs, the bleeding still deserves prompt assessment. A small amount, a single episode, or blood noticed only after sex should not be dismissed.

Ask where the bleeding appears to come from, whether your medication or hormone regimen could contribute, which examination or tests are suitable, and what follow-up is needed if the first tests do not explain it. Also ask how to respond if bleeding returns or becomes heavier.
