Black Hair Thinning at the Crown

The distribution and scalp surface offer useful clues. Gradual widening with finer hairs can occur in pattern hair loss, while increased shedding across the scalp can follow illness, major stress, nutritional problems, hormonal change, or medication exposure. Neither hair texture nor crown location confirms a diagnosis.

Male pattern hair loss commonly presents with progressive thinning at the crown or temples. Female pattern hair loss commonly presents with reduced density over the top of the scalp, often with the frontal hairline retained. Telogen effluvium commonly presents as increased shedding rather than one sharply defined patch.

At the crown, persistent tenderness, burning, itch, scale, bumps, shine, or fewer visible follicle openings can indicate an inflammatory scarring process. This deserves early review because controlling inflammation helps protect follicles that remain.

Take monthly photographs with the same hairstyle, part, distance, and lighting. Note excess shedding, scalp symptoms, recent illness, weight change, restrictive eating, pregnancy-related changes, new medicines, and relatives with patterned thinning. Reduce traction from tight braids, extensions, weaves, or heavy styles, and limit heat and harsh chemical processing.

A clinician may assess hair density, shaft breakage, follicle openings, and scalp inflammation, then order targeted tests if the history suggests an underlying trigger. Topical minoxidil can be considered for some pattern hair loss, while PRP for hair loss is a procedure discussed selectively. Scalp micropigmentation changes the appearance of density but does not regrow follicles.

Arrange an early dermatology visit for rapid progression, a smooth shiny area, loss of follicle openings, tenderness, burning, pustules, crusting, or spreading patches. Prompt assessment is also appropriate when shedding follows an unexplained systemic illness or comes with marked fatigue, menstrual changes, or other new symptoms.

Ask whether the pattern looks nonscarring or scarring, whether blood tests or a scalp biopsy would clarify the cause, and which styling changes matter most. If treatment is proposed, ask how to apply it safely, what side effects to watch for, and how progress will be documented.
