Bilateral ovarian cysts: what to know

The term describes location, not a single diagnosis or level of seriousness. Many ovarian cysts relate to the menstrual cycle, while other types may arise from endometriosis or different ovarian tissues. Their size, internal appearance, symptoms, and change over time guide the next step.

A report may call a cyst simple when it is thin-walled and filled with clear fluid, or complex when it contains solid-looking areas, partitions, or blood. Cysts on opposite ovaries do not have to be the same type. Menstrual status, pregnancy possibility, previous scans, and family or medical history add context.

Ovarian cysts can present with pelvic pain, pressure, bloating, pain during sex, or changes in bleeding, but some cause no symptoms. Symptoms cannot reveal the cyst type, so interpretation relies on clinical assessment and imaging rather than pain alone.

Discuss the report with a gynaecologist, including the measurements and appearance of each cyst. A transvaginal ultrasound may provide a closer view, and a repeat scan may be used to see whether a cyst resolves, remains stable, or changes. Keep a note of pain, bleeding, and cycle timing to share at the visit.

Treatment is based on the cyst type and your priorities. Observation may be appropriate for reassuring appearances. The combined pill can reduce the formation of some future cycle-related cysts, but it does not reliably remove an existing cyst. Persistent, enlarging, symptomatic, or concerning cysts may lead to discussion of laparoscopic ovarian cystectomy, with attention to preserving healthy ovarian tissue when feasible.

Go for urgent assessment if pelvic pain becomes sudden and severe, especially with nausea, vomiting, faintness, weakness, fever, or heavy vaginal bleeding. These symptoms can occur if an ovary twists, a cyst ruptures, or bleeding develops. Pain with a possible pregnancy also requires prompt evaluation for pregnancy-related causes.

Ask what type of cyst is suspected on each side, whether either has complex features, and when imaging should be repeated. Clarify how the cysts might affect fertility or ovarian reserve, which symptoms should trigger earlier review, and whether any proposed operation aims to remove the cyst while retaining the ovary.
