Bilateral hydrosalpinx: what the diagnosis means

The finding can make natural conception difficult because an egg and sperm may be unable to meet or a fertilised egg may not travel normally towards the uterus. It does not describe the ovaries or automatically mean that pregnancy is impossible.

Each fallopian tube normally provides a route between an ovary and the uterus. In hydrosalpinx, the far end of a tube is usually sealed, allowing fluid to collect inside. “Bilateral” simply specifies that this change is present on both sides.

Possible underlying damage includes previous pelvic infection, inflammation, endometriosis, or pelvic surgery. Pelvic inflammatory disease is one condition that can scar the tubes. Some people have pelvic discomfort or unusual discharge, while others first learn about hydrosalpinx during fertility testing.

Arrange a gynaecology or fertility consultation to confirm how the finding was made and whether further imaging or a tubal test is appropriate. Bring previous scan reports, details of pelvic infections or operations, and your pregnancy plans.

Management depends on symptoms, the condition of the tubes, and whether pregnancy is desired. Options may include observation, treating an active infection if present, salpingectomy to remove a badly damaged tube, or IVF, which allows fertilisation without relying on the tubes. Your clinician can explain how tubal fluid may influence an embryo-transfer plan.

Seek urgent medical assessment for severe or rapidly worsening lower-abdominal pain, fainting, shoulder-tip pain, heavy vaginal bleeding, fever, persistent vomiting, or feeling very unwell. If pregnancy is possible, pain or bleeding needs prompt evaluation because a damaged tube can be associated with a pregnancy developing outside the uterus.

Ask whether both tubes appear fully blocked, what may have caused the damage, whether testing for infection is needed, and how the finding changes your conception options. If a procedure is proposed, discuss what will happen to each tube, the expected recovery, and whether ovarian function should remain unchanged.
