Big pimples on the back: what they may be

Back skin has many oil-producing follicles. A blockage can become inflamed beneath the surface and form a tender nodule, while folliculitis creates inflamed bumps centred on hairs. Their treatment differs, so recurring or widespread eruptions merit a clear diagnosis.

Acne on the back may include blackheads, whiteheads and inflamed spots of different sizes. Folliculitis may look more uniform and can itch or become tender. Heat, sweat, oily products and friction from tight clothing, sports equipment or a backpack can aggravate either pattern without being the sole cause.

Deep lumps may remain under the skin after surface redness settles and can leave dark marks or indented scars. Sudden crops after starting a medicine or supplement should be discussed with the prescriber rather than stopping treatment independently.

Shower after heavy sweating, change out of damp clothing and choose loose, breathable fabric when practical. Wash gently and avoid abrasive body scrubs, picking and squeezing. Use non-comedogenic skin and hair products, and rinse conditioner from the back.

A clinician can match topical or oral treatment to the depth, spread and likely cause. Large nodules, scarring acne or persistent folliculitis often need more than cosmetic treatment. LED light therapy may be discussed as an adjunct for selected acne, but it does not replace diagnosis or a complete treatment plan.

Arrange a dermatology appointment for deep or repeatedly painful lumps, spreading outbreaks, drainage, scars or dark marks that continue to accumulate. Seek urgent medical care if redness spreads quickly, pain becomes severe, fever develops, or you feel unwell, because a deeper skin infection may need prompt treatment.

Ask whether the pattern fits acne, folliculitis or another eruption, and how the plan will prevent new scars. Bring a list of medicines, supplements and body products; mention itching, drainage, shaving, sports gear, sweating triggers and whether bumps occur elsewhere.
