A smooth, sharply outlined patch can occur with alopecia areata, while scale, broken hairs, tenderness, or itching can point toward a scalp disorder such as ringworm. Hair pulling, tight styling, friction, inflammation, and scarring conditions can also produce a visible gap.

The surface of the scalp offers useful clues. A smooth patch with normal-looking skin differs from an area that is red, flaky, crusted, shiny, scarred, or covered with short snapped hairs. Alopecia areata commonly presents with smooth, round or oval bald patches. Scalp ringworm may present with patchy loss alongside scale and fragile hairs, and it can spread between people.

Location and timing matter as well. Note whether the area appeared suddenly, is enlarging, or is accompanied by loss in other areas. A clinician may examine the scalp closely, gently test nearby hairs, and ask about styling, scratching, medicines, illness, and family history before deciding whether further testing is useful.

Arrange a non-urgent appointment with a dermatologist or primary-care clinician if the patch is new, unexplained, or growing. Until it is assessed, use gentle shampoo, avoid tight braids or extensions, limit heat and harsh chemical treatments, and do not pick at scale or crust. Take clear photographs in similar lighting so changes in the border are easier to track.

Do not share combs, hats, pillows, or hair tools if the scalp is scaly or itchy. Treatment depends on the diagnosis: immune-related loss, fungal infection, traction, and scarring inflammation require different approaches. Hair transplantation is not a first response to active or unexplained patchy loss; the underlying process should be identified and stable before restoration options are considered.

Seek prompt medical care if the area is painful, swollen, draining pus, rapidly expanding, or associated with fever or feeling unwell. A tender, boggy swelling or spreading redness can accompany infection and should not wait for routine cosmetic advice.

Book an early dermatology review if the skin looks shiny or scarred, the follicle openings seem absent, or there is burning around the edge. These features can occur when inflammation damages follicles, and earlier diagnosis may help protect hair that remains.

Ask what scalp features support the suspected diagnosis, whether a fungal sample or other test is needed, and how you will judge whether treatment is working. Also ask which grooming practices are safe, whether household contacts need assessment, and what change should trigger an earlier follow-up.
