Beginning signs of psoriatic arthritis

Symptoms may affect one joint or several and can be uneven between the two sides of the body. Psoriatic arthritis may occur in someone with psoriasis, including subtle scalp, navel or skin-fold patches, but skin and joint symptoms do not always begin together.

Inflammatory stiffness is often more noticeable after rest and may ease as the body gets moving. A finger or toe can swell along its full length. Pain may also occur where a tendon or ligament attaches to bone, particularly behind the heel or under it.

Nail pitting, lifting or thickening can accompany psoriasis-related disease. Lower-back or buttock stiffness can reflect inflammation around the spine or pelvic joints. None of these features alone confirms the diagnosis; clinicians assess the pattern, examine skin and nails, and consider imaging and blood tests to distinguish other causes.

Arrange a medical assessment when swelling, stiffness or tendon-area pain persists, returns or interferes with daily tasks. Note which joints are affected, whether symptoms follow rest, and whether a complete finger or toe swells. Photograph visible swelling, rashes or nail changes if they fluctuate before the appointment.

Share any personal or family history of psoriasis or inflammatory arthritis. Keep moving within comfort, but avoid repeatedly forcing a hot, swollen joint. Medication and exercise plans should match the working diagnosis; physiotherapy can help preserve movement and strength while medical treatment addresses inflammation.

A suddenly hot, very painful and swollen joint with fever or feeling acutely unwell needs urgent assessment because joint infection and crystal arthritis can resemble an inflammatory flare. Do not assume a new severe episode is psoriatic arthritis.

Seek urgent eye care for a painful red eye, light sensitivity or blurred vision. New weakness, loss of bladder or bowel control, or numbness around the groin requires emergency assessment. Otherwise, arrange a timely review for ongoing swelling or loss of function rather than waiting for several joints to become involved.

Ask which examination findings suggest inflammation, whether ultrasound or other imaging would clarify the cause, and what blood tests can or cannot exclude. Discuss how skin, nail, eye, bowel or back symptoms affect the assessment. If treatment is proposed, ask how response and medicine safety will be monitored.
