A beginning bald spot: what it may look like

A smooth round patch can occur with alopecia areata, while scale, broken hairs, itch, or tenderness can accompany scalp ringworm or another inflammatory problem. Gradual thinning at the crown may instead follow a patterned distribution.

Look at the border and the skin within the area. Note whether it is sharply outlined or diffuse, completely smooth or covered by short broken hairs, and whether there is redness, scale, crust, swelling, or scarring. Check for similar patches elsewhere and changes to eyebrows, eyelashes, or nails.

Alopecia areata commonly presents as smooth, clearly defined patches without surface scaling. Scalp ringworm may present with patchy loss plus scale and broken hairs, and it can spread between people. Appearance alone is not always enough to distinguish causes.

Photograph the spot with the surrounding scalp visible, using the same lighting and angle for later comparison. Avoid picking, close shaving over irritated skin, tight hairstyles, and unprescribed steroid or antifungal products, which can alter the appearance without addressing the correct cause.

Arrange an assessment with a doctor or dermatologist, particularly if the patch is expanding. Until a scaly or infectious cause has been excluded, do not share combs, hats, pillowcases, or hair tools. Hair-restoration surgery is not an appropriate starting point until the diagnosis and activity of hair loss are clear.

Seek timely care if the patch appeared suddenly, grows, multiplies, or comes with itching, scale, broken hairs, redness, pain, pus, or swollen glands. Children with a scaly bald patch should be assessed promptly because scalp fungal infection usually needs prescription treatment taken by mouth.

A tender, boggy swelling, drainage, fever, or rapidly worsening inflammation warrants urgent review. Also arrange evaluation for shiny scar-like skin or loss of visible follicle openings, because scarring processes can damage follicles if active inflammation continues.

Ask which diagnosis best fits the border, hairs, and scalp surface; whether magnified examination, hair sampling, or fungal testing is useful; and whether household contacts need attention. Discuss how treatment response will be monitored and what new change should bring you back sooner.
