BCC spots: understanding spots and pimples

People use “spot” for clogged pores, inflamed follicles, shaving bumps, facial rashes and skin growths. What matters clinically is the bump’s texture, location, timing and behaviour, together with symptoms such as itch, tenderness, crusting or bleeding.

Acne can present with blackheads, whiteheads and inflamed pimples in oil-rich areas. Hormonal acne often has a recurring pattern, while folliculitis produces inflamed bumps centred on hair follicles. Razor bumps follow hairs that curve back into the skin after close hair removal.

Rosacea may include central facial redness with acne-like bumps but usually lacks blackheads. Perioral dermatitis tends to form small clustered bumps around the mouth, nose or eyes. These conditions can resemble one another, and more than one may occur at the same time.

Use a gentle cleanser, avoid squeezing or picking, and pause new fragranced or oily products if they appear to aggravate the area. Record recent shaving, hair removal, medicines and skincare changes. If the eruption comes and goes, dated photos can preserve its distribution for the consultation.

Do not apply a strong steroid cream to an undiagnosed facial eruption unless it was prescribed for that problem; some pimple-like rashes can worsen or become harder to recognise. Treatment is chosen after the pattern and likely cause are identified.

Arrange an appointment if one spot persists, enlarges, repeatedly crusts or bleeds, looks unlike the surrounding outbreak, or fails to settle as the other pimples improve. These features merit examination even when the lesion does not hurt.

Seek urgent care if redness spreads quickly with marked warmth or swelling, if the area is near the eye and vision or eye movement is affected, or if a skin eruption occurs with breathing difficulty or swelling of the lips or tongue.

Ask whether the bumps arise from pores, follicles or another skin structure; which products should be stopped; and whether a swab, dermoscopy or biopsy would clarify the diagnosis. Also ask how to introduce treatment without irritating the skin and which changes should trigger reassessment.
