The Basics of Musculoskeletal Ultrasound

It can help assess tendons, muscles, ligaments, nerves, fluid collections, and the outer parts of joints. Because the image is live, the examiner can watch a structure move, compare the painful area with the other side, and ask where probe pressure reproduces symptoms.

You will usually sit or lie with the body part exposed. The clinician places gel on the skin and moves a handheld probe over the area. You may be asked to bend, straighten, rotate, or tense the limb so the tissue can be viewed during movement.

Ultrasound is particularly useful for superficial soft tissues, but it cannot see clearly through bone and has limited access to some deep structures. Image quality and interpretation also depend on the body region and the examiner's technique. Another type of imaging may therefore be needed when the clinical question lies beyond ultrasound's strengths.

Wear loose clothing that allows easy access to the painful area, and remove jewellery that covers it. Bring relevant reports or images if they were obtained elsewhere. Tell the examiner where symptoms occur and which movement brings them on, since this can guide a focused dynamic assessment.

Routine diagnostic musculoskeletal ultrasound usually needs no fasting or medication change. If the scan is planned alongside an injection, follow the clinic's specific instructions and disclose medicines that affect bleeding, allergies, infection symptoms, pregnancy, and recent procedures.

An ultrasound appointment is not a substitute for urgent assessment. Seek immediate care for a severely deformed limb after injury, a cold or blue extremity, sudden loss of feeling or power, an open wound over a suspected fracture, or rapidly worsening swelling with severe pain.

Contact a clinician promptly if the area becomes hot and red, drainage appears, fever develops, or you feel unwell. These features can require direct examination and treatment without waiting for routine imaging.

Ask which structure and diagnostic question the scan will target, whether movement will be assessed, and when the interpretation will be available. Afterward, clarify how the findings fit your symptoms, whether another test is needed, and how the result changes the treatment plan.
