Answer for the time period printed on the form and use the scale exactly as shown. Consider all relevant symptoms rather than only the worst moment. Complete it before an appointment if requested, and bring earlier results so your clinician can compare patterns.

Keep a brief note of medication changes, exercise, sleep disruption, and unusual physical demands. This context can explain a change and helps the care team decide whether physiotherapy, exercise planning, medication review, or further assessment fits your needs.

When to seek care

Arrange a clinical review if pain, stiffness, fatigue, or reduced movement is worsening, disrupting sleep, or limiting work and usual activities. Contact your care team if symptoms change substantially despite following the agreed plan; do not change prescribed medicine solely because of a questionnaire result.

Seek urgent assessment for new weakness or numbness in both legs, loss of bladder or bowel control, numbness around the groin, severe breathing difficulty, a painful red eye with light sensitivity, or major spinal pain after an injury.
