Why a bald spot may not be growing back

The absence of obvious regrowth does not reveal the cause by itself. The scalp surface, follicle openings, broken hairs, symptoms, and progression provide more useful clues.

Alopecia areata can present with smooth patches and may remain active even when the skin looks calm. Fungal infection can produce scale and break hair near the scalp. Ongoing traction, pulling, heat, or chemical injury may repeatedly damage emerging strands. In scarring forms of hair loss, inflammation can replace follicles with scar tissue; a smooth shiny area with fewer visible openings is an important clue. Fine pale regrowth can also be difficult to see without close examination.

Compare dated photographs taken with the same lighting and hairstyle. Record itch, soreness, scale, shedding elsewhere, and eyebrow, eyelash, or nail changes. Stop tight hairstyles and avoid scratching, strong chemicals, or home remedies that irritate the patch. A dermatologist can inspect the follicles under magnification and decide whether hair or scale sampling, blood tests, or a small skin sample would clarify the diagnosis. Therapy depends on whether the process is immune-related, infectious, traumatic, patterned, or scarring.

Arrange specialist review if a patch remains unchanged without regrowth, continues to widen, or develops a shiny texture. Seek earlier care for pain, persistent redness, heavy scale, crusting, or rapidly increasing hair loss. Pus, fever, spreading redness, or a tender boggy swelling requires prompt medical attention because infection may need prescription treatment.

Ask whether follicles are still visible, whether there is evidence of active inflammation, and whether the short hairs are broken or regrowing. Discuss what outcome is realistic, when response should be reassessed, and which changes would require an earlier return visit.
