Visible hair follicles in a bald spot: what they can mean

Dots, stubble, fine pale hairs, and broken shafts can look similar without magnification. Their significance depends on the patch border, skin surface, symptoms, and whether the area is changing.

A smooth patch with preserved openings can fit non-scarring hair loss. Alopecia areata commonly presents with a smooth, defined patch, although an examination is needed to distinguish it from other causes. Scale, itch, and hairs broken at different lengths can accompany scalp ringworm. Short regrowing hairs may indicate recovery, whereas repeated traction or pulling can also leave broken strands. A shiny surface where openings appear absent raises concern for scarring hair loss and deserves timely evaluation.

Use a well-lit photograph to record the patch rather than repeatedly tugging hairs to test them. Keep the scalp care routine gentle and pause tight braids, extensions, strong chemicals, or high heat over the site. If there is scaling, avoid sharing brushes, towels, headwear, or pillowcases until infection has been considered. A dermatologist may use magnification, examine loose hairs, or sample scale when the appearance alone is not enough.

Book a clinical assessment when the spot enlarges, additional areas emerge, or there is persistent itch, scale, redness, tenderness, or breakage. Prompt care is appropriate for pus, fever, spreading redness, marked swelling, or a painful boggy patch. Do not wait on visible follicles if the scalp is becoming shiny, scarred, or increasingly sore.

Ask whether the dots are open follicles, broken hairs, or early regrowth; whether there are signs of infection or inflammation; and whether scarring is present. Confirm how progress should be photographed and which topical products or styling practices could obscure the examination.
