The timeline matters as much as the position. Gradual thinning over the crown can follow a patterned distribution, whereas a patch that appears quickly calls for assessment of the scalp surface and remaining hairs. Burning, soreness, scale, crusts, bumps, or loss of follicle openings point toward an inflammatory, infectious, or scarring process rather than simple density change. A clinician may use magnification, a gentle hair-pull assessment, or a sample when infection is suspected.

Photograph the crown from the same angle under similar light, because overhead lighting can exaggerate visible scalp. Record when you first noticed the change and whether shedding, itch, pain, illness, new medicine, or a change in hair care occurred around the same time. Handle the area gently, avoid scratching and tight styling, and delay bleaching or chemical straightening if the scalp is irritated.

Book a clinician or dermatologist if the cause is unclear or the area is progressing. The correct treatment depends on whether follicles are miniaturising, inflamed, infected, or scarred. Hair transplant assessment is generally reserved for suitable stable hair loss after active scalp disease has been excluded or controlled.

Seek prompt medical advice if the patch expands rapidly, several patches develop, or there is pain, pus, marked swelling, spreading redness, fever, or a soft tender swelling. Arrange a dermatology visit for persistent burning, heavy scale, broken hairs, smooth shiny skin, or loss of eyebrows, eyelashes, or body hair. Earlier recognition matters when inflammation is damaging follicles because scarring can limit regrowth.

Useful questions include whether the change is true hair loss or breakage, whether follicle openings remain, and which diagnosis fits the distribution. Ask if scalp sampling, blood tests based on your wider symptoms, or photographs for comparison would add useful information. Confirm how the proposed treatment works, what scalp reactions to watch for, and when progress should be reassessed.
