Bald Spot on the Side of the Head

A smooth, sharply defined patch may occur with alopecia areata. Scale, broken hairs, redness, or itch can accompany scalp ringworm. Repeated pulling from tight hairstyles, friction from headwear, and local scarring or inflammation can also produce hair loss in this location.

Look closely without scratching the area. Note whether the scalp is smooth or scaly, whether hairs are missing from the root or snapped at different lengths, and whether there is tenderness, crusting, swelling, or discharge. A dermatologist may examine the scalp under magnification and check nearby hair, skin, and nails before deciding whether testing is useful. Location alone cannot identify the cause.

Take a clear photograph in consistent lighting so you can compare the border over time. Pause tight braids, extensions, close clips, and anything that repeatedly rubs the spot. Use gentle shampoo and avoid dyes, relaxers, strong oils, or unprescribed steroid creams on irritated skin. Do not share combs, hats, pillowcases, or towels if the patch is scaly or itchy, because a fungal scalp infection can spread between people.

Treatment depends on the diagnosis. Medicines used for an inflammatory patch differ from antifungal treatment, and scalp ringworm generally requires clinician-directed medicine rather than shampoo alone. Hair transplantation is considered only for selected stable forms of hair loss, not an unexplained or actively inflamed patch.

Arrange a medical assessment if the spot is enlarging, new patches appear, hair loss continues, or the scalp is itchy, scaly, painful, or inflamed. Seek prompt care for pus, marked swelling, spreading redness, fever, or a tender boggy area. Children with a scaly bald patch should be assessed promptly because prescription treatment may be needed and close contacts may require practical advice to limit spread.

Ask whether the follicle openings are still visible, whether the pattern looks inflammatory, infectious, traction-related, or scarring, and whether a fungal sample or other examination would clarify the diagnosis. Also ask which hair-care practices to stop, how to monitor regrowth, and what change should trigger an earlier follow-up.
