Itchy bald spot on the head: causes and care

Scalp ringworm is one possible explanation when patchy loss occurs with flaking or fragile hairs. Dermatitis, reactions to hair products and scratching can also damage shafts, while alopecia areata may occasionally cause tingling or itch despite a smooth surface. The combination does not identify a single cause without examination.

Loose scale, black-dot stubble, a widening edge or affected household contacts raise concern for a fungal scalp infection. Greasy flaking beyond the bald area can accompany dermatitis. A sharply outlined smooth patch with normal-looking skin can occur when alopecia areata presents as localized hair loss.

Recent dye, straightener, adhesive or fragranced oil use may trigger contact irritation. Scratching then worsens breakage and can introduce bacteria through damaged skin. Thick scale, pustules, a soft swollen patch or tender neck glands are more concerning than mild dry itch and warrant a clinician's assessment.

Avoid scratching, close shaving and new chemical products on the patch. Wash gently, rinse thoroughly and keep fingernails short. A cool compress can soothe irritation without obscuring the skin's appearance. Do not share brushes, hats, towels, pillowcases or hair tools when fungal infection is possible.

Seek a medical review rather than relying only on an anti-dandruff shampoo if hair is missing in a distinct patch. Scalp fungal infections generally need diagnosis and prescription medicine that reaches the hair follicle; surface creams alone may be inadequate. Bring product labels and photographs, and mention pets or close contacts with skin or scalp rashes.

Arrange prompt care for spreading redness, increasing pain, pus, heavy crusting, fever or a boggy swollen area. These signs may reflect substantial inflammation or a secondary infection. Rapid hair loss, new patches elsewhere, facial swelling, trouble breathing after a product, or involvement of a child also calls for timely professional assessment; breathing difficulty requires emergency help.

Ask whether the pattern looks fungal, inflammatory, allergic or autoimmune; whether scale should be sampled; whether close contacts need evaluation; and how to clean hair tools safely. Clarify which products can be continued, how to reduce itch without masking progression, and when regrowth should be reassessed.
