Bald spot on the head: what it may mean

A smooth round patch may occur with alopecia areata. Scale, itch or hairs broken close to the scalp can accompany a fungal infection, while repeated rubbing or pulling can cause breakage. An examination is the most reliable way to separate these possibilities and identify whether follicles are still healthy.

Notice whether the surface is smooth, flaky, red, crusted, tender or shiny. Check whether full-length hairs are absent or short stubble remains. One sharply edged patch differs from diffuse thinning, and a spot that enlarges at its border may need a different assessment from a stable area linked to an old injury.

Alopecia areata often presents with smooth patchy loss and may be accompanied by eyebrow loss or nail changes. Scalp ringworm can present with scaling and broken hairs and may spread between people. A scar, burn, inflammatory scalp disorder or sustained pressure can also disrupt growth in a defined area.

Photograph the spot from above with the hair parted the same way, then note when it appeared and whether it is changing. Keep grooming gentle, avoid scratching or picking, and stop any hairstyle or accessory that pulls at that location. Do not apply bleaching agents, essential oils or strong medicated products to broken skin.

Book a clinical assessment when the cause is unclear or the patch is progressing. A clinician may inspect the scalp with magnification, gently test nearby hairs and sample scale when infection is suspected. Blood tests are considered only when the history or wider pattern suggests a relevant health or nutritional issue. Management can range from removing a trigger to prescription treatment.

Get prompt care if the area becomes painful, swollen, hot, draining or covered by thick crust, especially with fever or feeling unwell. Arrange an earlier dermatology review for rapid spread, several new patches, loss of eyebrows or eyelashes, or skin that becomes shiny with fewer visible follicle openings. These features affect how quickly diagnosis and treatment should begin.

Describe scalp sensations, recent infections, medicines, grooming habits and similar hair loss in relatives. Useful questions include whether the hairs are shedding or breaking, whether there is evidence of inflammation or scarring, whether testing is necessary, and what improvement or worsening should look like during follow-up.
