An itchy bald spot: what it may mean

Itch is a useful clue rather than a diagnosis. Scale, broken hairs, a ring-shaped edge or tender bumps can accompany scalp ringworm. A smooth patch without obvious scale can occur with alopecia areata, although some people notice tingling or itch around the time hair is lost.

White or greasy flakes may fit dandruff or dermatitis, while crust, pus or painful swelling raises concern for infection. Hairs broken close to the scalp can make an area appear bald even when follicles remain present. Hair products, dyes, adhesives and tight styles can also produce itching followed by breakage or shedding.

Scratching can worsen inflammation and introduce bacteria through damaged skin. Because several conditions overlap in appearance, trying multiple medicated products at once can hide useful signs and irritate the scalp further.

Avoid scratching, close shaving, fragranced oils, harsh dyes and tight headwear over the patch. Use a mild shampoo and lukewarm water, then pat rather than rub the area dry. Clean combs and clippers, and avoid sharing them when scale or infection is possible.

Photograph the border and note flakes, sores, discharge, recent products, animal contact and whether anyone at home has scalp symptoms. A pharmacist or clinician can advise on suitable care, but a suspected scalp fungal infection often needs diagnosis-specific treatment rather than an ordinary anti-dandruff product.

Get prompt medical care if the area becomes boggy, very painful, hot, swollen or drains pus, or if fever or spreading redness develops. Arrange an appointment soon if the patch expands, additional spots appear, hairs keep snapping, or itch continues despite stopping new products. Scalp changes in a child should be assessed early when ringworm is possible because it can spread to others.

Ask whether the surface pattern points to fungus, dermatitis, alopecia areata or mechanical damage; whether hairs or scale should be sampled; and which products to stop. Clarify how to reduce household spread if infection is diagnosed and what changes would show that treatment is working.
