The spot's surface and location offer useful clues, but appearance alone cannot confirm the cause. A smooth, sharply defined patch can occur with alopecia areata, while scale, broken hairs, redness or tenderness points toward a scalp process that needs assessment.

Clues that narrow the possibilities

Notice whether hair is missing from the root or snapped at different lengths. Also check for flaking, pustules, crust, burning or a widening part. Alopecia areata commonly presents as one or more smooth bald patches. Scalp ringworm can present with patchy loss plus scale and fragile hairs. Tight braids, extensions, ponytails or head coverings secured under tension can gradually affect the hairline or other pressure points.

Recent illness, major stress, childbirth, medication changes and symptoms such as fatigue or menstrual changes may matter, although these often produce more widespread shedding rather than one isolated circle.

What to do now

Take a clear photograph in consistent lighting and note when the patch appeared and whether it is enlarging. Pause tight styles, chemical processing and direct heat on the area. Wash gently, avoid scratching, and do not share combs, scarves or pillowcases if there is scaling or a possible infection.

Arrange a clinician or dermatologist visit if the cause is unclear. Examination of the scalp and hairs may be enough; sometimes a fungal sample, blood tests or a small scalp sample is considered. Treatment depends on the diagnosis, so steroid creams, antifungal products and hair-growth medicines should not be treated as interchangeable.

When medical care should be prompt

Seek prompt assessment for pain, pus, marked swelling, spreading redness, fever, rapidly expanding loss, or a shiny scar-like surface. Book a routine appointment when the patch persists, new patches appear, eyebrow or eyelash hair is affected, or scalp symptoms interrupt sleep. Children and anyone with a suspected contagious scalp infection should be assessed rather than treated only with cosmetic camouflage.
