Do not judge the cause from the gap alone. A clinician will consider its border, whether short hairs remain, how the skin feels and whether other areas are affected. Treatment is selected for the identified problem; cosmetic filling products can camouflage the area but do not address inflammation, infection or an immune-related pattern.

With alopecia areata, facial hair loss can present as a round or oval smooth area. Folliculitis can leave tender bumps or pustules around hairs, while contact irritation may cause burning, redness and flaking after a new dye, oil or fragrance. Fungal involvement is more plausible when there is scale and broken hair, but visual clues can overlap. Loss with smooth shiny skin and few visible follicle openings raises concern for follicle damage.

Leave the patch alone rather than repeatedly checking it by pulling hairs. If trimming, disinfect the tool and use a guard to reduce friction. Choose a mild cleanser and stop any recently introduced product that causes discomfort. Record the date and outline of the spot, then arrange an assessment if it does not settle or begins to enlarge. Bring product labels and photos to help connect skin changes with exposures.

Medical review should be prompt when redness spreads beyond the hair-bearing area, pain increases, pus develops or you feel unwell. A rapidly multiplying group of smooth patches also merits timely assessment, including a check of scalp hair, eyebrows and nails. Seek urgent help for breathing difficulty or sudden swelling around the mouth. A stable patch without discomfort is not usually an emergency, but a dermatologist can still clarify its cause.

Consider asking whether the appearance fits shedding, breakage or scarring, and whether a fungal sample or other test would change management. Confirm how long to use any medicine and which side effects on facial skin should prompt a call. If regrowth begins, ask what texture or colour changes can occur so normal early hairs are not mistaken for treatment failure.
