Bald spots at the front hairline

Look beyond the amount of visible scalp. A gradually changing outline with increasingly fine hairs differs from a sudden, smooth gap. Broken fringe hairs, redness, scale, bumps, burning or a shiny surface provide further clues about follicle stress or scalp disease.

Patterned hair loss often changes the overall front outline or temples over time. Traction tends to affect hairs under repeated pull and may leave a fringe of short or sparse strands. Alopecia areata can present with a sharply bounded smooth patch that interrupts an otherwise unchanged hairline.

Inflammatory and scarring disorders may cause burning, tenderness, redness, scale or reduced follicle openings. Scalp ringworm can present with scaly areas and broken hairs, although its appearance varies. A diagnosis should rely on scalp examination rather than location alone.

Loosen braids, ponytails, extensions, wig attachments or headwear that grips the same front area. Minimise edge-control products, vigorous brushing and high heat if hairs are snapping. Protect exposed skin from sun with shade or a comfortable hat that does not rub the patch.

Take a photograph with the hair gently held back and repeat it under matching conditions. If the gap persists or grows, arrange an examination. A dermatologist can assess miniaturisation, breakage, follicle openings and inflammation and decide whether sampling or laboratory tests are relevant.

Seek a timely appointment for rapid expansion, more patches, eyebrow loss, persistent burning or a smooth shiny band at the edge. These features warrant evaluation for an active condition rather than prolonged cosmetic concealment.

Painful swelling, pus, thick crust, spreading redness or fever calls for prompt care. New loss after a medicine change, illness or marked dietary restriction should also be discussed, even if the scalp surface appears normal.

Ask whether the process is active and whether follicles remain visible in the patch. Find out which styling changes are essential, what treatment targets the suspected cause and how response will be judged. If you are considering a hair transplant, first ask whether the loss is stable and the recipient scalp is healthy.
