Bald spots in men: causes, checks and next steps

A slowly enlarging thin area at the crown may fit male-pattern hair loss, while a sharply defined smooth patch can be a presentation of alopecia areata. Scale, redness, tenderness, broken hairs or discharge point toward a scalp disorder that deserves examination rather than cosmetic treatment alone.

Location and surface clues help separate causes. Patterned loss tends to develop gradually at the temples, front or crown, with finer hairs around the thinning zone. Alopecia areata can present with one or more smooth, clearly outlined patches. Scalp ringworm can present with patchy loss alongside scaling, itch and hairs broken close to the skin.

Friction from headwear, tight styling, scratching or pulling may also damage shafts or follicles. A clinician may inspect the scalp closely, test how firmly nearby hairs are anchored and ask about medicines, illness, nutrition and family hair patterns.

Photograph the area in consistent light and note whether it is widening, shedding, itching or becoming scaly. Use gentle shampoo, avoid close shaving over irritated skin and pause tight hairstyles or repeated rubbing. Do not share combs, clippers, hats or towels if scaling or broken hairs raise concern for a contagious fungal infection.

Arrange a primary-care or dermatology visit when the cause is unclear or the spot is progressing. Treatment depends on the diagnosis; a hair transplant is not a first step for an active inflammatory, infectious or rapidly changing condition.

Seek prompt assessment for pain, swelling, pus, crusting, spreading redness or fever, as these features may accompany infection or marked inflammation. Book an appointment soon if loss appears suddenly, several new patches develop, eyebrows or beard hair are affected, or the exposed skin looks shiny and scar-like.

Also mention fatigue, unexplained weight change, restrictive eating or a recently started medicine because these details can change the evaluation.

Ask whether the hair shafts are miniaturising, breaking or missing from the follicle; whether the scalp shows inflammation or scarring; and whether fungal testing or blood tests would clarify the cause. Discuss what improvement would look like, how progress should be photographed and which scalp products could interfere with treatment.
