A Bald, Itchy Spot on the Scalp: Causes and Warning Signs

Scale, broken hairs, tenderness, a smooth surface or an enlarging edge can help distinguish possible causes. Avoid assuming that itch alone identifies the diagnosis.

Scalp ringworm can present with itchy, scaly areas, broken hairs, crusting or a tender swollen patch. It can spread to other people and needs antifungal treatment chosen for scalp involvement. Alopecia areata more often presents as a clearly defined smooth patch, although some people notice tingling or itch around changing areas.

Scratching, tight hairstyles, chemical products and inflammatory scalp disorders can also damage hairs or make an existing patch more noticeable. A clinician may examine the scalp closely, gently test nearby hairs or sample scale when infection is suspected.

Use gentle shampoo, keep nails short and avoid scratching, tight headwear, harsh chemicals and close shaving over the patch. Do not share combs, clippers, towels, hats or pillowcases while a fungal infection remains possible. Clean grooming items and wash fabrics that contact the scalp.

Take clear photographs to track the border and note whether hairs are falling from the root or snapping close to the skin. Avoid starting leftover steroid cream because it can change the appearance of some infections and delay correct treatment.

Arrange a medical review for an enlarging or persistent bald patch, broken hairs, scale, crusting or contact with someone who has a fungal infection. Seek prompt care for a boggy or very tender swelling, pus, fever, rapidly spreading redness, or painful neck glands. Early assessment matters when inflammation is intense because prolonged damage can affect regrowth.

Ask whether the hairs look broken or shed from the root, whether a fungal sample is appropriate, whether close contacts need checking, and what scalp products are safe while treatment begins. Mention similar patches elsewhere, nail changes, recent hair services, shared clippers and contact with pets.
