The bagel sign and ectopic pregnancy

It describes a ring-like mass in the adnexa, the area containing the ovary and fallopian tube. The scan findings are interpreted alongside symptoms, examination, pregnancy-hormone results, and whether a pregnancy is visible inside the uterus.

On transvaginal ultrasound, the sign appears as a bright ring of tissue surrounding a darker central area, separate from the ovary. This may represent pregnancy tissue implanted in a fallopian tube. A corpus luteum within the ovary can also look ring-like, so establishing the mass's location is essential.

Ectopic pregnancy can present with pelvic or abdominal pain, vaginal bleeding, shoulder-tip pain, dizziness, fainting, or no obvious symptoms early on. The diagnosis is based on the combined picture; the presence or absence of a single sign should not be interpreted at home.

The maternity or gynaecology team will assess pain, bleeding, blood pressure, pulse, examination findings, ultrasound images, and pregnancy-hormone tests. If the situation is not yet clear and you are clinically stable, repeat blood tests or another scan may be needed to locate the pregnancy.

Management depends on symptoms, scan findings, hormone trends, the size and location of the ectopic pregnancy, and your overall health. Options can include close monitoring, medicine, or surgery such as removal of the affected fallopian tube. The team should explain warning signs and exactly where to return if symptoms change.

Seek emergency care immediately if pregnancy is possible and you develop severe or worsening abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, collapse, breathing difficulty, or heavy vaginal bleeding. These can signal internal bleeding from a ruptured ectopic pregnancy.

Do not drive yourself if you feel faint or acutely unwell. Call local emergency services, say that you may have an ectopic pregnancy, and follow the urgent-care plan given by the treating unit even if an earlier scan was inconclusive.

Ask whether the mass is clearly separate from the ovary, whether a pregnancy is visible in the uterus, and what the hormone trend indicates. Clarify which management options are suitable, how each may affect the fallopian tube and future pregnancy, what follow-up is required, and which symptoms mean returning immediately.
