When freckles deserve a closer look

Freckles are small, flat areas of increased pigment that often become darker after ultraviolet exposure and fade when exposure decreases. The word “bad” may describe a cosmetic concern, a sudden increase after sun, or worry that one mark is not truly a freckle.

A typical freckle is flat and fairly uniform in colour. A dermatologist should assess a mark that develops an irregular outline, several colours, persistent itching or tenderness, spontaneous bleeding, crusting, rapid enlargement, or a noticeably different appearance from nearby spots.

New pigmentation after strong sun exposure does not identify the type of lesion on its own. A clinician may examine the skin with magnification and compare the mark with the rest of your pattern. Cosmetic treatment should wait until any uncertain lesion has been identified.

Reduce ultraviolet exposure with shade, protective clothing, a hat and broad-spectrum sunscreen used as directed. Reapply after swimming or heavy sweating. Do not try to burn, bleach or scrape freckles away, and avoid unregulated lightening mixtures that may irritate or discolour skin.

Check your skin in consistent lighting and note a spot’s location and appearance so genuine change is easier to recognise. For unwanted but clinically assessed pigmentation, a dermatologist can discuss light-based or laser options, expected fading, irritation risk and the possibility that pigment may return with sun exposure.

Arrange an appointment for any evolving or unusual pigmented mark, particularly one that bleeds without injury or remains sore, crusted or unhealed. Seek timely review rather than treating it cosmetically first. A widespread blistering sunburn, confusion, fainting or marked illness requires urgent care for the burn itself.

Ask whether each targeted mark is suitable for cosmetic treatment, how your skin tone affects the chance of lightening or darkening afterward, and what sun protection is needed. Clarify the expected recovery signs and which changes should trigger a follow-up.
