Short answer

A lump at the back or side of the neck should be assessed if it persists, enlarges, or comes with concerning symptoms.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common tissue
Lymph nodes can enlarge in response to infections of nearby areas
Avoid
Repeated squeezing or pressing, which can irritate the area
Prompt review
A persistent, enlarging, hard, fixed, or unexplained lump

The short answer

Possible explanations include a reactive lymph node after infection, a skin cyst, an inflamed hair follicle, or a soft fatty lump. Head and neck cancer, lymphoma, thyroid cancer, or spread from another site can also present with a neck mass, but the lump itself does not establish a diagnosis.

Clues from location and behaviour

A clinician checks whether the lump sits in the skin or deeper tissues, whether it is tender, firm, mobile, warm, or attached, and whether there are several enlarged nodes. Recent throat, ear, dental, or scalp infection can be relevant because lymph nodes drain these nearby areas.

A painless mass that keeps growing, does not settle, or accompanies persistent hoarseness, swallowing difficulty, an unexplained mouth sore, night sweats, fever, or weight loss merits prompt investigation. None of these features should be used alone to label the cause.

How to respond

Avoid repeatedly squeezing or pressing the area. Note when it appeared, whether its size changes, recent infections, skin changes, dental problems, pain, and any swallowing or voice symptoms. Arrange a clinical examination if it remains after an illness has resolved, grows, feels hard or fixed, or has no clear explanation.

Assessment may include inspection of the scalp, mouth, throat, ears, thyroid, and skin. Depending on the findings, the clinician may request imaging or take a needle sample rather than relying on touch alone.

When to get urgent help

Seek emergency care if neck swelling develops quickly and affects breathing or swallowing, or if it follows a severe allergic reaction. Urgent assessment is also appropriate for a rapidly enlarging painful swelling with fever, spreading redness, drooling, a muffled voice, marked stiffness, or severe illness.

What to ask during assessment

Ask which tissue the lump seems to arise from, whether nearby infection could explain it, what examination or scan would clarify the cause, whether sampling is needed, and what change in size or symptoms should bring you back sooner.

Questions people ask

Can an infection cause a lump behind the neck?

Yes. Lymph nodes may enlarge while responding to infections involving the scalp, ear, throat, teeth, or nearby skin. A lump that does not settle still needs review.

Does a painless neck lump mean cancer?

No. Painless lumps have several possible causes. Because some cancers can present with a painless neck mass, persistence or growth should prompt medical assessment.

Will a clinician need to biopsy the lump?

Not every lump requires sampling. The decision depends on its location, examination features, duration, associated symptoms, and any imaging findings.
