A lump at the back or side of the neck

Possible explanations include a reactive lymph node after infection, a skin cyst, an inflamed hair follicle, or a soft fatty lump. Head and neck cancer, lymphoma, thyroid cancer, or spread from another site can also present with a neck mass, but the lump itself does not establish a diagnosis.

A clinician checks whether the lump sits in the skin or deeper tissues, whether it is tender, firm, mobile, warm, or attached, and whether there are several enlarged nodes. Recent throat, ear, dental, or scalp infection can be relevant because lymph nodes drain these nearby areas.

A painless mass that keeps growing, does not settle, or accompanies persistent hoarseness, swallowing difficulty, an unexplained mouth sore, night sweats, fever, or weight loss merits prompt investigation. None of these features should be used alone to label the cause.

Avoid repeatedly squeezing or pressing the area. Note when it appeared, whether its size changes, recent infections, skin changes, dental problems, pain, and any swallowing or voice symptoms. Arrange a clinical examination if it remains after an illness has resolved, grows, feels hard or fixed, or has no clear explanation.

Assessment may include inspection of the scalp, mouth, throat, ears, thyroid, and skin. Depending on the findings, the clinician may request imaging or take a needle sample rather than relying on touch alone.

Seek emergency care if neck swelling develops quickly and affects breathing or swallowing, or if it follows a severe allergic reaction. Urgent assessment is also appropriate for a rapidly enlarging painful swelling with fever, spreading redness, drooling, a muffled voice, marked stiffness, or severe illness.

Ask which tissue the lump seems to arise from, whether nearby infection could explain it, what examination or scan would clarify the cause, whether sampling is needed, and what change in size or symptoms should bring you back sooner.
