How a burn scar can affect the back
Healing after a deeper burn can produce excess, firm collagen. A hypertrophic scar stays within the burn boundary but may rise, redden, itch, or feel tender. A keloid extends beyond the original injury. Flat scars may instead leave lighter or darker colour.
A broad scar across the upper back can tighten as it matures and restrict shoulder movement. Texture, colour, pliability, sensitivity, and range of motion are therefore assessed separately; improving one feature does not necessarily correct the others.
Care and treatment options
Once the skin has fully closed, regular moisturising can reduce dryness and massage may be advised to help mobility. Silicone gel or sheets can support raised-scar care, while clothing and broad-spectrum sunscreen help limit colour contrast from ultraviolet exposure. Never place scar products on an open or weeping burn unless the treating team directs it.
A specialist may use corticosteroid injections for a raised scar, fractional laser treatment for selected texture or flexibility concerns, or scar revision when tension and shape can be improved surgically. Skin grafting is generally considered when tissue coverage or release of a tight contracture is needed.
Changes that need medical assessment
Have the area assessed if it repeatedly cracks, bleeds, forms an ulcer, drains fluid, becomes increasingly painful, or develops spreading redness and warmth. A new lump or persistent wound within an old burn scar also warrants direct examination.
Prompt specialist review is appropriate when tightness limits shoulder or arm movement. Early rehabilitation advice can address posture and mobility before restriction becomes harder to manage.
