Typical Meniere's disease involves episodes of spinning vertigo with fluctuating hearing loss, ringing or other ear noise, and a sense of fullness in the affected ear. In an atypical presentation, hearing symptoms may appear without clear vertigo, balance attacks may dominate, or the features may evolve over time.

Assessment commonly includes a detailed episode history, ear and neurological examination, and a hearing test.
