Atrial fibrillation and hypertension

Long-standing high blood pressure makes the heart pump against greater resistance. The heart muscle may thicken, and the upper chambers can stretch or develop scarring. Those structural changes can disrupt electrical signalling and support atrial fibrillation.

Both conditions may be silent. Hypertension is identified through reliable blood-pressure measurements, while atrial fibrillation requires rhythm assessment, usually with an ECG. When they occur together, blood pressure forms part of the overall evaluation of clot-related stroke risk and cardiovascular health.

Use a validated upper-arm monitor if home checks are recommended. Sit quietly, support the arm, use the correct cuff size and record readings as instructed. Bring the log and your device to an appointment so the technique and monitor can be checked.

Take blood-pressure, rate-control and rhythm medicines exactly as prescribed. Some drugs affect both pulse and pressure, so dizziness or weakness after starting or increasing one deserves review. Regular activity, reduced excess salt, avoiding tobacco, limiting alcohol and addressing sleep apnoea can complement medication. Anticoagulation is a separate decision based on clot and bleeding risks, not on the blood-pressure reading alone.

Get emergency help for chest pain, severe breathlessness, fainting, sudden facial droop, one-sided weakness, speech difficulty or a sudden major change in vision. These warning signs require immediate assessment regardless of the home monitor result.

Arrange timely clinical advice for repeated readings outside your personalised range, a persistently unusual pulse, new palpitations, or light-headedness after a treatment change. Do not take extra tablets to chase a single reading unless your clinician has provided a specific written plan.

Discuss your individual blood-pressure and heart-rate targets, the best timing for home measurements, and whether any medicines could be simplified. Ask how kidney function and electrolytes will be followed, whether an ECG or longer rhythm monitor is needed, and how your stroke-prevention decision was reached.
