Why the conditions interact
Atrial fibrillation removes the coordinated squeeze of the upper heart chambers and may make the lower chambers beat too quickly or unevenly. That can reduce efficient filling and pumping. If heart failure is already present, this rhythm change may bring more breathlessness, fatigue or swelling.
The relationship also works in the other direction: pressure and stretching within a heart affected by heart failure can make atrial fibrillation easier to develop or sustain. Symptoms overlap, so an examination, ECG and often blood tests or heart imaging help show what has changed.
Managing both conditions
Take prescribed medicines consistently and keep a current medication list. Treatment may slow the ventricular rate, restore or maintain rhythm, relieve fluid congestion and support the heart's pumping function. A separate assessment determines whether anticoagulation is appropriate for protection from clots and stroke.
Track changes that reflect congestion or reduced circulation: increasing ankle or abdominal swelling, needing more pillows to breathe comfortably, waking short of breath, unusual exhaustion, dizziness or reduced ability to walk. Follow any personalised plan for fluid, salt, activity and monitoring; do not independently stop a diuretic, rate-control medicine or anticoagulant.
Signs that need prompt assessment
Call emergency services for severe breathing difficulty at rest, chest pressure, fainting, blue or grey lips, confusion, or sudden weakness or speech difficulty. These symptoms can reflect poor circulation, a heart emergency or stroke and should not be watched at home.
Contact your care team promptly if swelling, breathlessness, palpitations or fatigue is clearly worsening, or if dizziness starts after a medicine change. Early review can identify fluid build-up, an unsuitable heart rate, low blood pressure or another trigger such as infection.
Make the care plan clear
Ask whether the immediate priority is rate control, rhythm control or both; how heart function will be monitored; and which symptoms should activate your action plan. Confirm whether anticoagulation is advised, which medicines require blood tests, and whom to contact when swelling or breathing changes.

Does atrial fibrillation always cause heart failure?
No. The conditions can occur separately, although a persistently unsuitable heart rate can strain the heart and existing heart failure can promote atrial fibrillation.
Why can breathing worsen when atrial fibrillation starts?
An irregular or rapid rhythm can reduce how efficiently the heart fills and pumps. In someone with heart failure, that may increase pressure behind the heart and contribute to breathlessness.
Is anticoagulation a treatment for heart failure symptoms?
No. Anticoagulation reduces clot-related stroke risk in selected people with atrial fibrillation; it does not directly remove excess fluid or strengthen pumping.
