Asthma triple therapy inhalers: what to know

It is generally considered when asthma remains troublesome despite regular controller treatment. It is taken on a schedule and is not automatically a substitute for the separate reliever medicine in your written asthma plan.

The inhaler contains an inhaled corticosteroid, a long-acting beta agonist and a long-acting muscarinic antagonist. The corticosteroid calms airway inflammation; the other medicines relax airway muscles through different pathways. Keeping them in one device can simplify a prescribed maintenance routine.

This treatment does not cure asthma. A clinician first checks the diagnosis, inhaler technique, regular use, triggers and other problems that can worsen control. Possible effects vary by medicine and can include a sore mouth, hoarse voice, dry mouth, tremor or a racing heartbeat.

Use the device exactly as prescribed, even on days without symptoms. Ask for a hands-on technique check because the correct breath differs between device types. If the product contains a corticosteroid, rinse your mouth and spit after the dose. Track night waking, activity limits and reliever use so your clinician can judge control.

Do not stop or change doses by yourself. Know which inhaler is for daily control and which one your plan directs you to use during sudden symptoms. Arrange a medication review if doses are missed, the device is difficult to operate, or side effects interfere with treatment.

Seek urgent medical help for severe breathlessness, difficulty speaking in full sentences, blue or grey lips, confusion, drowsiness, marked chest pulling, or symptoms that keep worsening despite following the emergency steps in your asthma plan. Use prescribed rescue treatment while help is being arranged.

Contact your clinician promptly if wheezing, chest tightness or night waking is increasing, or if the reliever is needed more often than your plan expects. These changes can signal poor control and call for assessment rather than simply taking extra maintenance doses.

Ask what each component does, whether the same inhaler is ever used for relief, how to recognise an empty device, and which side effects should prompt a call. Request a written action plan and demonstrate your technique during the appointment.
