What to know about an asthma maintenance inhaler

Many maintenance inhalers contain an inhaled corticosteroid that reduces swelling and sensitivity inside the airways. Some combine this medicine with a long-acting bronchodilator that helps keep the airway muscles relaxed. The effect builds through consistent use, so the inhaler may not produce an immediate sensation of relief.

A maintenance inhaler is not automatically the same as a rescue inhaler. Certain combination inhalers can serve both purposes under a maintenance-and-reliever plan, but only when specifically prescribed that way. The label, written asthma action plan, and clinician's instructions should make its role clear.

Take each scheduled dose as directed and do not stop simply because symptoms have settled. Ask a doctor, nurse, or pharmacist to watch your technique with your own device; the breathing method differs between aerosol and dry-powder inhalers. A spacer may be recommended for some aerosol devices.

If the inhaler contains a corticosteroid, rinse your mouth with water and spit it out after use to reduce hoarseness, soreness, and oral thrush. Keep track of remaining doses, replace the device before it is empty, and bring all inhalers to asthma reviews. Record night waking, activity limits, wheezing, chest tightness, cough, and reliever use because these details help show whether control is slipping.

Arrange a prompt clinical review if symptoms are waking you, limiting normal activity, becoming more frequent, or requiring more reliever medicine than your action plan allows. Also seek advice for persistent mouth soreness, white patches, troublesome hoarseness, shaking, or a racing heartbeat after inhaler use.

Get emergency help for severe breathlessness, difficulty speaking in full sentences, bluish or grey lips, confusion, exhaustion, rapidly worsening symptoms, or poor response to the reliever steps in your asthma action plan. Sit upright, use the prescribed reliever as your plan directs, and do not rely on extra maintenance doses unless the plan specifically says the same inhaler is also your reliever.

Ask which medicine is inside the device, whether it is maintenance-only or also a reliever, and what to do after a missed dose. Request a technique check and a written asthma action plan. It is also useful to clarify which symptoms should trigger a routine review, an urgent appointment, or emergency care, and when treatment can be stepped down safely under supervision.
