What big toe arthritis means

Big toe arthritis, also called hallux rigidus, affects the joint where the big toe meets the foot. Loss of smooth joint movement can make the toe stiff and cause pain on top of the joint, especially when the heel lifts during walking. A bony prominence may also rub against shoes.

Treatment cannot be chosen from an X-ray alone. A clinician also checks where the toe hurts, how far it moves, whether footwear presses on the joint and whether pain comes from another part of the foot.

Choose shoes with a wide toe box and a firm or rocker-shaped sole to reduce bending at the painful joint. Custom orthotics or a stiff insert may redistribute pressure. Adjust activities that repeatedly force the toe upward, while maintaining fitness with options that do not provoke symptoms.

A clinician may discuss suitable pain medicine or a cortisone injection for temporary symptom control. If pain remains limiting, surgery may remove obstructing bone, preserve selected joint motion or fuse the joint. Joint fusion stops movement at the arthritic joint but can provide a stable platform for walking. The choice reflects the joint damage, desired activities and tolerance for recovery.

When to seek care

Arrange an assessment if toe or foot pain persists, shoes are becoming difficult to wear, or stiffness changes the way you walk. Seek prompt care after an injury if the toe looks deformed, you cannot bear weight, or numbness or colour change develops.

A suddenly hot, red and very swollen joint, particularly with fever or feeling unwell, needs urgent medical evaluation because infection and crystal arthritis require different treatment from routine osteoarthritis care.
