Antibiotics when a suspected spider bite becomes cellulitis

The original mark may be a bite, scratch, inflamed follicle, or small abscess. The clinician focuses on spreading inflammation, the presence of pus, and how unwell you feel. Mild cellulitis may be managed with oral medicine, while severe illness can require hospital treatment.

Cellulitis presents with enlarging warmth, tenderness, swelling, and red or discoloured skin. On darker skin, the affected area may look purple, grey, or more deeply brown rather than bright red. Fever can accompany cellulitis when the infection is affecting the body more widely.

A soft or draining centre can suggest an abscess, which may need a procedure to release pus. An actual spider exposure is often difficult to confirm, so photographs of the spider are useful only when they can be taken without approaching or handling it.

Use the antibiotic exactly as prescribed and tell the clinician about allergies, pregnancy, kidney problems, recent antibiotics, and other medicines. Rest and elevate an affected arm or leg when practical. Gently clean broken skin, apply the recommended dressing, and avoid squeezing, cutting, or attempting to drain the area yourself.

Observe the border of discolouration and the intensity of pain. Arrange review if either continues to advance, if a new pus-filled centre develops, or if the treatment causes a concerning reaction. Treating cracks between the toes, including athlete's foot when present, can remove a route for bacteria entering the leg.

Seek urgent care for rapid spread, severe or escalating pain, fever with shaking chills, red streaks, vomiting, confusion, faintness, fast breathing, or unusual sleepiness. Blisters, numbness, black or dusky skin, and pain that seems far greater than the visible change require emergency assessment.

Do not wait for dramatic redness if you have type 2 diabetes, poor circulation, reduced skin sensation, or weakened immunity. These factors can obscure early changes and complicate infection.

Ask whether there is a drainable abscess, when the area should be rechecked, and what signs would justify hospital care. Confirm how to manage pain, protect the skin opening, handle a missed dose, and respond if swelling or fever appears after starting treatment.
