Over-the-counter antacids and the combined pill

Antacids neutralise acid in the stomach and can affect how some medicines are absorbed, which is why their labels may advise spacing doses. The product ingredients, rather than the word antacid alone, determine whether separation is needed. Follow the packet directions for both medicines and avoid exceeding the stated dose.

A separate issue arises if you vomit soon after taking an active combined pill or have severe diarrhoea. The pill may not have been fully absorbed, so contraceptive protection can be reduced. This is an absorption problem caused by illness, not evidence that ordinary heartburn itself changes the pill.

Show the pharmacist the exact antacid and combined-pill packets, along with any supplements or other medicines. Use the spacing instructions printed on the antacid label. If vomiting or severe diarrhoea occurs, follow the missed-pill and sickness guidance in your contraceptive leaflet; the advice varies with when active pills were affected and where you are in the pack. Ask promptly about emergency contraception if unprotected sex occurred during a period when protection may have been reduced.

Seek urgent care for chest pressure, trouble breathing, fainting, vomit containing blood, or black sticky stools. Persistent difficulty swallowing, repeated vomiting, unexplained weight loss, severe abdominal pain or indigestion that keeps returning also needs clinical assessment. If you take the combined pill, sudden breathlessness, chest pain, coughing blood, or painful swelling in one leg requires urgent evaluation for a possible blood clot rather than treatment with an antacid.

Ask whether the ingredients in your indigestion remedy need to be separated from the combined pill or other regular medicines. If symptoms recur, discuss whether another medicine, a medical condition or the timing of meals could be contributing. Clarify exactly what missed-pill action applies if sickness affected an active tablet.
