Anovulation and no period: what it can mean

Without ovulation, the usual rise and fall of progesterone does not occur. The womb lining may therefore shed unpredictably, remain in place for longer, or cause spotting rather than a recognisable period.

A missing period does not identify the cause by itself. Pregnancy should be excluded first when it is possible. Other explanations include polycystic ovary syndrome, changes in thyroid or prolactin hormones, marked stress, changes in weight or exercise, some medicines, and reduced ovarian function.

Anovulation can also make conception difficult because there is no egg available for fertilisation during that cycle. Bleeding patterns alone cannot confirm whether ovulation occurred, so an assessment may include your cycle history, a pregnancy test, blood tests and, when appropriate, pelvic ultrasound.

Record bleeding, spotting, pelvic symptoms, medicines, weight changes and home ovulation test results if you use them. Take a home pregnancy test after a missed period if pregnancy is possible; repeat it or arrange clinical testing if the result is unclear and bleeding has not started.

Book a routine appointment if periods repeatedly disappear, cycles remain unpredictable, or you are trying to conceive. Treatment depends on the reason and your goals. It may address an underlying hormone problem, protect the womb lining, regulate bleeding, or stimulate ovulation when pregnancy is desired.

Seek urgent care for severe or worsening pelvic pain, fainting, marked dizziness, shoulder-tip pain, or heavy bleeding that rapidly soaks through period products, particularly if pregnancy is possible. These symptoms need prompt assessment rather than waiting for a period to return.

Arrange timely medical review for a positive pregnancy test with pain or bleeding, milky nipple discharge when not breastfeeding, new severe headaches or vision changes, or symptoms of significant thyroid disturbance such as a racing heartbeat with tremor.

Ask which causes fit your pattern, whether pregnancy testing, hormone tests or ultrasound would help, and whether the womb lining needs protection if bleeding stays absent. If conception is your goal, discuss how ovulation will be assessed and when fertility treatment might be considered.
