The short answer

Calendar timing alone cannot show whether an ovary released an egg. A clinician can assess ovulation using the cycle history and, when appropriate, hormone testing or ultrasound.

Why bleeding may still look regular

Oestrogen can thicken the uterine lining even when ovulation does not occur. The lining may then become unstable and shed at intervals that resemble a period. Because no egg was released, the usual rise in progesterone after ovulation is absent.

Anovulation may be occasional or persistent. Possible contributors include polycystic ovary syndrome, thyroid or prolactin problems, marked changes in weight or exercise, stress, some medicines, and changes in ovarian function. Regular bleeding neither proves nor rules out any one cause.

How to check what is happening

Record the first day, flow and length of each bleed, along with pelvic pain, acne, new facial hair, hot flushes, nipple discharge and medicines. Home ovulation tests detect a hormone surge but do not by themselves prove that an egg was released.

If confirmation matters, particularly when trying to conceive, book a gynaecology visit. Testing may include pregnancy testing, blood tests timed to the cycle and an ultrasound. Management is directed at the cause and your goals rather than bleeding dates alone.

When to seek care

Arrange a routine assessment if you have difficulty becoming pregnant, bleeding between periods, a new change in flow, or symptoms such as hot flushes, milky nipple discharge or increased facial hair. Seek urgent care for very heavy bleeding with faintness, severe or worsening pelvic pain, or bleeding with a positive pregnancy test.

Ask whether your bleeding pattern is consistent with ovulation, when any hormone test should be taken, whether ultrasound would help, and whether medicines or health conditions could be affecting the cycle.
