Clinicians look beyond the location. Pain linked with inflammatory spinal disease may wake a person during the latter part of sleep, worsen with prolonged inactivity, and improve after movement. Mechanical pain from a strain may follow a different pattern, but symptoms can overlap.

Ankylosing spondylitis commonly presents with spinal or buttock pain, but pain in any one of these regions does not establish the condition. Examination can separate joint restriction, tendon-attachment tenderness, muscle pain, and nerve-related symptoms; blood tests or imaging may then clarify the picture.

Track and manage the pattern

Use a body map to mark pain and note whether it moves, switches sides, or accompanies swelling. Record night waking, morning stiffness, activity response, fatigue, eye irritation, skin changes, and bowel symptoms before a medical review.

Keep gently active within tolerable limits and break up long periods of sitting. A physiotherapist can adapt spinal mobility, posture, chest-expansion, and strengthening exercises. Medication choices should be reviewed with a clinician because suitability depends on other health conditions and current medicines.

Pain and red flags

Obtain urgent assessment for a painful red eye, sensitivity to light, or blurred vision. These are possible features of uveitis, which can accompany ankylosing spondylitis and should not wait for the next routine visit.

Emergency evaluation is needed for severe spinal pain after trauma, new limb weakness, groin numbness, loss of bladder or bowel control, chest pain, or severe difficulty breathing. New unexplained fever with intense back pain also needs prompt medical attention.

Questions for a pain review

Ask whether the distribution looks inflammatory, which joints or tendon attachments are involved, and whether any symptom suggests nerve compression. Discuss how to modify exercise during a flare and what outcome will show that treatment is helping.

Can ankylosing spondylitis pain change sides?

Yes. Inflammation around the sacroiliac joints can produce alternating buttock pain, although changing sides is not diagnostic by itself.

Can rib pain be related?

Inflammation where the ribs meet the spine or breastbone can cause chest-wall discomfort and make deep breathing feel restricted. Sudden chest pain or breathing difficulty needs urgent assessment.

Does heel pain fit the condition?
